  • Incontinence care and supplies
    • $1.00 per Disposable Undergarment
    • $5.00 per day service charge

·         Assistance with ADL’s (e.g. toileting, dressing, undressing)

·         Physician ordered treatments (e.g. foot soaks, wound care)

·         Beautician/Barber Services

    • Men’s Haircut: $10.00
    • Women’s Wash & Set: $17.00
    • Permanent: $50.00 (cut included)
  • Transport to medical/dental appointments
    • Provided by Bryan Manor
    • $40.00 per round-trip
    • $10.00 per hour waiting

·         Name

·         Social Security Number

·         Date of Birth

·         Billing Address

·         Previous phone number

·         A toll number that was called on the previous bill

·         Signatures (with date) of service recipient (where applicable) and/or Guardian/Representative
